Structured observation to explore systems awareness and patient safety learning in undergraduate nursing education: A mixed-methods educational study.

Background

Preparing nursing students for contemporary practice requires more than technical skill acquisition. It demands the ability to interpret complex organisational environments, recognise system-level risks and engage with patient safety as a shared professional responsibility. However, during clinical placements, learning often remains task-focused, with limited pedagogical support to foster systems awareness and clinical management understanding.

Aim

To examine the perceived educational utility of a structured observation strategy in relation to systems awareness, clinical management understanding, and patient safety competencies among undergraduate nursing students during clinical placements.

Design

A descriptive, cross-sectional mixed-methods educational study.

Methods

Undergraduate nursing students undertaking clinical placements in three hospitals and nine primary care centres completed the Clinical Management Observation Tool (CMOT), comprising ten Likert-scale items addressing key domains of clinical management and patient safety culture, and one open-ended reflective question. Quantitative data were analysed descriptively and comparatively across settings, while qualitative responses were examined using inductive thematic analysis. Although descriptive in design, the study was conceived as an exploratory evaluation of a pedagogical strategy rather than a causal effectiveness study.

Results

A total of 125 students participated. Overall scores indicated positive observations of organisational functioning and patient safety culture across settings. Primary care settings were rated significantly higher than hospital settings in patient and family information and participation. Qualitative findings revealed students‘ capacity to identify organisational strengths, system pressures and realistic improvement opportunities related to teamwork, communication, workflow and leadership.

Conclusions

Structured observation functions as a low-cost educational intervention that supports reflective, systems-oriented learning in complex clinical environments. By guiding students to critically interpret organisational processes rather than focus solely on task performance, structured observation may contribute to the development of professional judgement, patient safety awareness and readiness for contemporary nursing practice.
